Our mission is to help individuals live as fully as possible wherever they call home by providing comfort, dignity, and respect through high-quality primary, palliative, and hospice care. Our rapidly growing home-based primary and palliative care program serves a panel of 600+ patients and continues to expand due to strong community demand. This is an excellent opportunity to join a stable, mission-driven team with experienced clinicians and strong medical leadership. We are seeking an experienced Nurse Practitioner or Physician Assistant to provide home-based primary care, palliative care, and limited hospice support for adult and geriatric patients.
