**Licensed Practical Nurse (LPN) - Forestdale, AL**

**Shifts Available: 2:30-11:00pm Monday-Friday (NO WEEKENDS)**

**and Baylor Positions (Weekends) 7pm-7am - GREAT FOR COLLEGE STUDENTS!**

**Job Summary:**

Birmingham Rehabilitation and Wellness Center is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals. As an integral part of our interdisciplinary team, the LPN will provide high-quality patient care and support in a dynamic and fast-paced rehabilitation setting.

**Responsibilities:**

- Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and performing vital signs
- Develop and implement individualized care plans in collaboration with the interdisciplinary team
- Maintain accurate and up-to-date patient records and documentation
- Participate in patient education and discharge planning to ensure a smooth transition to home or community-based care
- Collaborate with physical, occupational, and speech therapists to ensure comprehensive patient rehabilitation
- Maintain a safe and clean environment for patients, staff, and visitors

**Requirements:**

- Current LPN licensure in the State of Alabama
- CPR Certification
