## Licensed Practical Nurse (LPN)

**Full-Time & Part-Time | Lake Pointe Health & Rehabilitation | Conneaut, OH**

We are currently seeking **Licensed Practical Nurses (LPNs)** to join our **night shift team** at our **Skilled Nursing Facility in Conneaut, Ohio**. We have **2 Full-Time and 1 Part-Time openings** for compassionate, dependable nurses who are committed to high-quality resident care.

### Position Details

- Shift: Night Shift – 7:00 PM to 7:30 AM
- Employment Type: 2 Full-Time | 1 Part-Time
- Location: Conneaut, OH (serving Ashtabula County and surrounding areas)

### LPN Responsibilities

- Provide direct nursing care to residents in accordance with care plans
- Administer medications and treatments as prescribed
- Monitor residents’ condition and report changes to RN or physician
- Accurately document care in electronic medical records
- Collaborate with CNAs and nursing team to ensure resident safety
- Maintain compliance with state and federal regulations

### Qualifications

- Active Ohio LPN license (required)
- Experience in skilled nursing or long-term care preferred
- Strong clinical and communication skills
- Ability to work independently on night shift
