### Your Role

- Provide compassionate, high-quality care to residents in our skilled nursing facility

- Administer medications and treatments as prescribed and within your LPN scope of practice

- Collaborate with RNs, CNAs, and other team members to ensure coordinated, person-centered care

- Monitor residents’ conditions and promptly report changes to the charge nurse or Director of Nursing

- Promote restorative care to encourage residents’ independence and well-being

- Maintain accurate and complete documentation of all nursing care provided

- Support and assist fellow nursing staff to maintain a positive, patient-centered environment

- Ensure resident comfort, safety, and dignity while upholding facility standards

### Who We’re Looking For

- Current Licensed Practical Nurse (LPN) license in the state of Illinois

- Graduate of an accredited nursing program

- Compassionate and committed to delivering quality, personalized care

- Team-oriented, reliable, and professional
