Clinical Social WorkerJob SummaryProvides individuals, families, and groups with the support needed to cope with chronic, acute, or terminal illnesses.General AccountabilitiesCollaborates with medical professionals to evaluate patients' medical or physical conditions and to assess client needs.Refers patients to community resources to assist in recovery.Counsels patients to help them overcome dependencies, recover from illness, and adjust to life.Monitors, evaluates, and records client progress according to measurable goals described in treatment and care plan.Modifies client treatment plans to comply with any changes in status.Organizes support groups or counsels family members to help them understand, deal with, and support the patient.Attends and participates in staff development programs, department related conferences and administrative meetings.*The company reserves the right to add or change duties at any time.Job QualificationsEducation: Master's degreeExperience: Appropriate certifications and/or licensures with at least 5 years of related experienceSkillsExcellent verbal and written communication
