**Job Title:** Registered Nurse (RN) – Home Health & Hospice

**Position Type:** Full-Time / Part-Time / PRN

**Job Summary:**

We are seeking a compassionate, dedicated, and professional **Registered Nurse (RN)** to join our Home Health and Hospice team. The RN will provide skilled nursing care to patients in their homes or hospice settings, ensuring high-quality clinical outcomes, patient safety, and comfort. The ideal candidate demonstrates clinical expertise, empathy, and strong communication skills while promoting patient-centered care and interdisciplinary collaboration.

**Key Responsibilities:**

- Conduct comprehensive initial and ongoing patient assessments, including physical, psychosocial, and environmental evaluations.
- Develop individualized care plans in collaboration with physicians, patients, and families.
- Administer medications, IV therapies, wound care, and other skilled nursing procedures as ordered.
- Monitor, record, and report patient progress and changes in condition to physicians and other care team members.
- Provide education to patients and families regarding disease management, medications, and home care techniques.
- Coordinate patient care with interdisciplinary teams, including therapists, social workers, and home health aides.
- Ensure accurate and timely documentation in the Electronic Medical Record (EMR).
- Participate in on-call rotation as assigned.
- Maintain compliance with all state, federal, and agency policies, procedures, and regulations, including HIPAA.
- Provide compassionate end-of-life care and emotional support to patients and families in hospice settings.

**Qualifications:**

**Education & Licensure:**

- Current and valid Registered Nurse (RN) license in the State of Texas (or compact state).
- Graduate of an accredited School of Nursing (BSN or ADN).
- Current CPR (BLS) certification required.
- Valid Driver’s License, auto insurance, and reliable transportation required.

**Experience:**

- Minimum one (1) year of clinical nursing experience preferred (home health or hospice experience strongly preferred).
- Knowledge of OASIS and EMR charting preferred for home health nurses.
- Hospice RNs should have knowledge of symptom management, palliative care, and end-of-life protocols.

- Ability to work independently and manage time efficiently.
- Proficient in computer systems and electronic documentation.

**Working Conditions:**

- Patient visits in home, assisted living, and hospice settings.
- Requires driving to patient homes and facilities.
- Flexible schedule including possible weekends and on-call duties.
- Must be able to lift, bend, and assist patients as needed.
