Job Description
Improving Health for Children and Families Ronald McDonald House New York (RMH-NY) is partnering with AmeriCorps to expand access to care and improve health outcomes for medically complex children and their families. As part of New York State's Medicaid Waiver initiative, RMH-NY is seeking AmeriCorps members to serve as Patient Navigators. In this role, Patient Navigators will perform telephone outreach to families eligible for Medicaid to: • Conduct screenings for Health-Related Social Needs (HRSNs) • Facilitate referrals and connect families to essential services • Provide care coordination in collaboration with hospital partners, Social Care Networks (SCNs), and community-based organizations These efforts will strengthen RMH-NY's capacity to help New York families achieve better health by supporting them across medical and social systems of care.
This is a full-time (1,700 hours) or half-time (900 hours) AmeriCorps opportunity for one year, beginning in late October 2025. Members receive a living allowance and education award, with stipends of $25,384 (full-time) or $13,483 (half-time) for the full term of service. Through this experience, members will gain professional training in population health and social service navigation while making a direct impact on the lives of children and families facing complex health challenges.
Members will also have opportunities to participate in Ronald McDonald House New York's many additional activities for sick children and families. Member Duties : In this role, AmeriCorps Members will be Patient Navigators supporting the Community Health Workers by performing telephonic outreach to families with medically complex children in NYC. Members will: • Conduct screenings for Health-Related Social Needs (HRSNs) • Facilitate referrals and connect families to essential services • Provide care coordination in collaboration with hospital partners, Social Care Networks (SCNs), and community-based organizations By working directly with community members, you will help them achieve better health outcomes.
