Your Role & Responsibilities:
1. Comprehensive Clinical Assessments
- Conduct health and wellness assessments for aged care residents.
- Identify and manage acute and chronic conditions proactively.
- Perform polypharmacy reviews to minimise adverse drug interactions.
- Conduct psychotropic medication reviews in line with best practices.
- Prescribe, adjust, and deprescribe medications as clinically indicated.
- Manage residents with diabetes, hypertension, COPD, and dementia.
- Develop and implement personalised care plans to improve outcomes.
- Coordinate with GPs and allied health professionals for optimal management.
- Assess and manage chronic and complex wounds.
- Prescribe wound care treatments and implement infection control measures.
- Provide symptom management and comfort-focused interventions.
- Collaborate with families and facility staff to ensure best practice palliative care.
- Perform ear examinations, iron infusions, and routine blood tests.
- Order and interpret pathology and imaging results as needed.
- Participate in weekly TeleRounds, discussing high-risk residents.
- Collaborate with care managers, RNs, and allied health teams for continuity of care.
- Document all assessments and interventions in eCase or facility records.
- Ensure timely updates to GPs, aged care staff, and families.
- Provide clinical education and mentorship to aged care staff where needed.
- Liaise with facility managers to identify gaps in clinical care.
- Recommend best practice care models and improvements.
- Assist in staff training, workflow optimisation, and quality improvement initiatives.
