Registered Nurse Case Manager Qualifications

• Active and valid RN license in the State of Michigan
• Bachelor of Science in Nursing (BSN) degree required
• Minimum 2 years of case management nursing experience (utilization case management experience not applicable)
• Strong interpersonal, communication, and negotiation skills
• Proficient in analytical thinking, data management, and PC skills
• Excellent organizational and time management abilities; able to prioritize multiple tasks
• Ability to work independently and exercise sound clinical judgment
• Preferred: Experience with Care Manager and Epic EHR systems, discharge planning, and 3+ years of RN clinical experience

Case Management RN Responsibilities

• Collaborate with physicians and interdisciplinary healthcare team members to facilitate patient care and improve clinical outcomes
• Coordinate utilization management, patient education, care facilitation, and discharge planning for assigned patient caseload
• Plan and manage patient care to promote efficient resource utilization and meet individual patient needs
• Facilitate precertification and payor authorization processes for healthcare services
• Work across the continuum of care, intervening to remove barriers to timely and efficient care delivery
• Apply process improvement methodologies to evaluate and enhance outcomes of care
• Support clinical documentation and serve as a resource for coders to ensure accurate healthcare documentation
• Coordinate communication with physicians and other members of the healthcare team
• Demonstrate nursing knowledge and skills necessary to provide care for patients across the lifespan
