What You’ll DoAs a Case Worker, you will:Provide rehabilitative visits in community settings(homes and neighborhoods), focusing on: Activities of daily living (ADLs)Coping and emotional regulation skillsSocial integration and community engagementBudgeting, transportation, and health navigationMeet or exceed the 5‑visit‑per‑month standard through thoughtful, individualized engagementCoordinate care with therapists, prescribers, and community partners, documenting collateral contacts and care planningSupport and track person‑centered goals (IRPs) and complete accurate, timely documentation in the EHRCollaborate closely with your Rehabilitation Specialist and supervisor to ensure quality services, compliance, and strong clinical outcomes What You BringWe’re looking for someone who has:Experience supporting adults with serious mental illness (SMI) or significant behavioral health needs (community‑based work preferred)Strong communication, organization, and follow‑through in a mobile, field‑based roleComfort with documentation, EHRs, and weekly visit planning, with attention to detail and quality notesA valid driver’s license and reliable transportationAbility to work in assigned service areas (AA, HoCo, PG, Baltimore City, Baltimore County) Nice‑to‑Have (Not Required)CPRP certification or progress toward certificationPrior PRP experienceBilingual skills Schedule & Work EnvironmentMobile / field‑based positionFlexible scheduling with some TelehealthWeekly supervision and ongoing team collaborationSupportive leadership that values growth, balance, and professional development Ready to Grow With Us?This spring, plant yourself in work that makes a difference—every day, in real lives, across our communities.Apply today and be part of something meaningful.
