Job DescriptionJob DescriptionCase WorkerLocation: New York, NY 10016Duration: 8 WeeksSchedule & Assignment DetailsSchedule: 5 days per weekHours: 7 hours per dayWeekly Hours: 35 hoursPosition Overview"Navitas Healthcare, LLC" is seeking a compassionate and organized Case Worker to provide social services support and case management assistance to individuals and families in need of public assistance, medical support services, institutional care, and protective services.The role involves assessing client needs, developing individualized service plans, determining eligibility for support programs, and coordinating access to community resources to promote positive client outcomes.Key ResponsibilitiesCase Management & Social ServicesProvide support to individuals and families accessing public assistance and community resourcesConduct comprehensive client assessments to identify social, economic, medical, and environmental needsDevelop, implement, and monitor individualized service plansDetermine eligibility for public assistance, food assistance, medical assistance, and related programsConnect clients with appropriate community resources and support servicesClient Support & CounselingProvide guidance, advocacy, and supportive counseling to clients and familiesAssist clients in navigating social service and healthcare systemsConduct follow-up and investigatory activities related to case management servicesMaintain regular communication with clients, families, and external agenciesDocumentation & ComplianceMaintain accurate and timely case records and documentationEnsure compliance with agency policies, procedures, and regulatory requirementsMonitor case progress and update service plans as neededParticipate in interdisciplinary meetings and case reviewsRequired QualificationsEducation:Bachelor’s Degree from an accredited college or university (required)Experience:Minimum of 3 years of related case work or social services experience (required)Language:Strong proficiency in English (speaking, reading, and writing)For more details contact at [email protected] or Call / Text at 516-862-1169.About Navitas Healthcare, LLC certified WBENC and one of the fastest-growing healthcare staffing firms in the US providing Medical, Clinical and Non-Clinical services to numerous hospitals.
