Case Worker

Job DescriptionJob DescriptionPromise Point Care is seeking a compassionate and dedicated Case Worker to support and empower youth in a structured, trauma- informed environment. The Case Worker will play a vital role in helping youth develop life skills, achieve personal goals, and successfully transition into independent living.

Key Responsibilities

Manage a caseload of youth and provide ongoing support servicesDevelop and implement individualized service/treatment plansConduct assessments and monitor client progress regularlyAdvocate for youth needs and coordinate with external agencies (schools, counselors, courts, etc.)Maintain accurate and up-to-date case documentation and reportsProvide guidance, mentorship, and crisis intervention when needed Facilitate life skills development (education, employment readiness, coping skills)Qualifications

Experience in social work, human services, or working with youthExperience with at-risk youth preferredStrong communication and organizational skillsAbility to build trust and maintain professional boundariesCompassionate, dependable, and team-orientedAbility to handle sensitive situations with professionalismWork Environment
