Position Overview
The Family Nurse Practitioner (FNP) provides comprehensive, patient-centered primary care services to individuals experiencing homelessness. This role focuses on delivering high-quality, trauma-informed care in shelter-based clinics, outreach settings, and/or community health centers. The FNP will address acute and chronic medical conditions while considering the complex social, behavioral, and environmental factors impacting this population.
Key Responsibilities
- Provide comprehensive primary care services, including assessment, diagnosis, treatment, and follow-up care across the lifespan
- Manage acute and chronic conditions such as hypertension, diabetes, asthma, and infectious diseases
- Deliver preventive care services including physical exams, immunizations, screenings, and health education
- Conduct outreach and engage patients in non-traditional settings (e.g., shelters, street outreach, mobile clinics)
- Prescribe medications and treatments in accordance with state regulations and organizational protocols
- Collaborate with interdisciplinary teams including social workers, case managers, behavioral health providers, and outreach staff
- Address social determinants of health, including housing instability, food insecurity, and access to benefits
- Provide harm reduction education and support (e.g., substance use, overdose prevention)
- Maintain accurate and timely documentation in the electronic health record (EHR)
- Participate in care coordination, case conferencing, and discharge planning
- Ensure compliance with all regulatory, organizational, and clinical guidelines
Qualifications
- Master’s or Doctorate degree from an accredited Nurse Practitioner program
- Current, unrestricted Nurse Practitioner license in New York
- Board certification as a Family Nurse Practitioner (FNP-BC or FNP-C)
- DEA registration and prescriptive authority
