Position Status: Full-Time (40-hours per week)
Position Schedule: The standard schedule is Monday through Friday, hours will vary based upon client and program needs
Salary Range: $21- $26.66/hour, based on education and experience
Make a Difference in the Lives of Children and FamiliesGA Family Services is seeking a compassionate, dedicated, and motivated Post-Adoption Program Case Worker to support children, adoptive families, and guardianship families throughout Chautauqua, Cattaraugus, Erie, Wyoming, and Niagara Counties.This grant-funded position through the New York State Office of Children and Family Services (OCFS) plays a vital role in helping families build stability, strengthen relationships, and access critical resources that promote long-term success and well-being.If you are passionate about trauma-informed care, family support, advocacy, and community collaboration, we encourage you to apply.Position OverviewThe Post-Adoption Program Case Worker provides comprehensive case management and support services to children and families involved in post-adoption and post-guardianship services. This role focuses on strengthening family functioning, promoting permanency and stability, and enhancing each child’s ability to thrive at home, in school, and within the community.The Case Worker manages an assigned caseload and works closely with families, schools, service providers, and community partners to ensure individualized and effective support.
QualificationsBachelor’s Degree in Human Services or a related field required; Master’s Degree preferredMinimum of two years of direct service experience working with children and families preferredValid driver’s license, reliable transportation, and vehicle insurance as required by New York State lawStrong communication, organizational, and interpersonal skillsExperience working with children and families preferredKnowledge of trauma-informed and strength-based practices preferredBilingual in English and Spanish preferred, but not requiredKey ResponsibilitiesConduct screenings, assessments, and ongoing case management for post-adoption and post-guardianship familiesDevelop and maintain individualized service plans focused on family stability and child well-beingProvide support, education, advocacy, and crisis intervention to children, caregivers, and familiesCollaborate with schools, community providers, and interdisciplinary team members to coordinate servicesConnect families with counseling, behavioral health, and community resourcesMaintain accurate and timely documentation, reports, and case records in compliance with agency and OCFS requirementsParticipate in team meetings, supervision, trainings, and on-call responsibilitiesSupport agency initiatives, outreach efforts, and evolving program needs
