The Role We're looking for an experienced Care Navigator to join our premium, highly personalised digital health startup. This is a pivotal, patient-facing role that sits at the heart of our clinical and operational delivery. You'll be the first person our patients turn to — guiding them through their care, supporting them on our platform, and helping us build something we're all proud of.
This isn't a back-office role. It requires clinical credibility, genuine warmth, and the kind of startup mindset that thrives when things are still being figured out. What You'll Do Be the face (and voice) of Added Health Patient Care Act as the first and primary point of contact for patients, delivering a warm, professional and empathetic experience at every interaction.
Triage and coordinate patient queries, appointments and follow-ups, escalating clinical concerns appropriately to GPs and clinical leads. Research and signpost patients to services in their area as part of our white-glove care model. Maintain accurate, up-to-date patient records in line with clinical and regulatory standards.
Operations Support the development and continuous improvement of operational workflows and standard operating procedures. Use our proprietary AI-enabled software platform for running the service; and provide feedback for improvements Contribute to the smooth day-to-day running of the practice with strong organisational and prioritisation skills. What We're Looking For Essential Clinical background as a Nurse Practitioner, Allied Health Professional, or equivalent, with 5–10 years of hands-on experience.
A genuine sense of patient empathy — able to make patients feel heard, supported and at ease, particularly in a digital-first care environment. Excellent interpersonal and communication skills — personable, professional and able to build trust quickly with patients and senior clinicians alike. Comfortable in a technology-enabled environment; able to quickly learn and become proficient in bespoke software platforms.
Experience working within NHS and/or private or concierge healthcare settings. Clear understanding of patient confidentiality, CQC regulations and GDPR. Proven ability to manage administrative and operational tasks alongside clinical responsibilities.
