Base pay range

$130,000.00/yr - $160,000.00/yr

Employer: 21st Century Home Health Services (21HHS) – the largest home health agency in San Francisco with 600+ employees, serving more than 4,000 patients across multiple Bay Area counties and expanding into Marin and Sonoma counties.

Note: All opportunities require field visits to patients in their homes. Remote/work-from-home positions are not available unless noted.

Responsibilities

• Duties and responsibilities include, but are not limited to, providing hands-on patient care within RN license scope (e.g., PICC lines, wound care, NG/G/J tube management, drains, Foley/suprapubic catheters, Trach care, blood draws, ostomy management, injections, IV antibiotics/TPN, and related procedures).
• Provide direct patient care as defined by the state Nurse Practice Act.
• Implement and document a plan of care initiated and directed by the Case Management RN.
• Document accurately and timely within 24-48 hours of care, consistent with the plan of care.
• Educate patients and families/caregivers about diagnoses and plans of care.
• Assist the Case Management RN with skilled procedures and prepare equipment for treatments while helping patients learn self-care techniques.
• Coordinate home health services and report identified needs to Case Management RN or other disciplines (HHA, OT, PT, MSW, ST).
• Use equipment and supplies efficiently; participate in ongoing professional growth.
• Attend weekly Case Conferences as required; ensure patient privacy and maintain medical records with professional standards.
• Maintain productivity standards, complete Start of Care assessments, and participate in on-call duties as needed.

Qualifications

• One (1) year of recent nursing experience (experience requirement may be waived for internal candidates and/or new grad postings).
• One year of home health care experience is preferred.
• Experience with high-acuity patients is a plus; experience working with an interdisciplinary team is highly desired.
• Graduate of an accredited school of nursing (Bachelor's degree).
• Current CA Registered Nurse License (RN).
• Current Basic Life Support (BLS) Card.
• Valid CA Driver's License with acceptable driving record.
• All licenses and certifications must be current at time of hire and sustained throughout employment.
• Self-directed, critical thinker with excellent interpersonal, organizational, communication, and problem-solving skills; basic math skills; nursing skills per competency checklist.
• Must stay current with annual compliance training and certifications.

Compensation details: Estimated wage range for Home Health Registered Nurse (RN) is $130,000 - $160,000. Starting pay is based on relevant experience above the minimum requirements. Compensation follows a pay-per-visit model with the wage provided as an estimate. 21st Century offers comprehensive health benefits and a 401k plan with up to a 4% match.
