Come join our team where you can make a difference every day!Responsibilities:deliver professional nursing services in client's home setting and provide case management for primary patientscomplete initial assessment and plan of care for home care clients prepare clinical and progress notes, submitting all required paperwork dailycoordinate services, informing the physician, management, and other personnel of changes in the client's condition and needsensure quality and accuracy of patient records and charts in compliance with Medicare and Medicaid standardsdemonstrate compassion while working with ill, disabled, elderly and/or emotionally disturbed patientsrespond promptly to telephone calls and emailsparticipate in continuing education programs to stay current on home health practices, laws and regulationspost hospital visitsmonthly client visitshelp to maintain confidentiality filesmedication management servicesperform other duties as assignedEducation and Experience Requirements:RN with current MA license, BSN preferredone or more years of current nursing experience in certified home care is preferred, but not requiredability to sit, stand, bend, lift, kneel, climb stairs and move patients safely as requiredability to work in the patient's home environment as well as the officedemonstrate strong verbal, written and interpersonal skills to communicate effectivelymust have reliable automobile, valid driver's license, and current auto insurancefluent in English with skills in a second language a pluscomputer skills sufficient to capture notes and required documentation Job Posted by ApplicantPro
