Responsibilities

• Management of patient population to optimize utilization.
• Discharge planning.
• Coordinate with physicians, nurses, physical therapy and ancillary staff to facilitate all departmental processes.
• Communicate with Home Health Agencies and Managed Care Plans, provide clinical information for admission and follow-up patient reviews.
• Ensure payer documentation and information requirements are met.

Requirements

• New York state License for Registered Professional Nurse and current registration or valid permit to practice in New York state as an R.N.
• Bachelor's degree required.
• Minimum of 2-4 years of experience required in an acute care setting.
• Knowledge of Medicare Regulations.
• Excellent interpersonal and communication skills.
• Experience with electronic health records.
• Case Management Certification preferred (CCM).
