We are seeking a compassionate and skilled Part-Time Speech Language Pathologist to join our home health team. The Speech Language Pathologist will provide high-quality, patient-centered speech and language therapy services to individuals in their homes. This role focuses on evaluating and treating communication and swallowing disorders, while collaborating with patients, families, and interdisciplinary care teams to support optimal outcomes.
Key Responsibilities:
- Conduct comprehensive assessments of speech, language, cognitive communication, and swallowing disorders in a home setting
- Develop individualized treatment plans based on patient needs, goals, and physician orders
- Provide evidence-based therapeutic interventions to improve communication, cognition, and/or swallowing function
- Educate patients, caregivers, and family members on therapy techniques, home exercises, and safety strategies
- Monitor patient progress and adjust treatment plans as needed
- Maintain accurate, timely documentation in compliance with regulatory and organizational standards
- Collaborate with physicians, nurses, occupational therapists, physical therapists, and other healthcare professionals
- Participate in care planning meetings and case conferences as required

- Valid New Hampshire State Licensure SLP issued by the NH Office of Professional Licensure & Certification (OPLC).
- ASHA Certification: Master’s degree or higher in Speech-Language Pathology (SLP); ASHA Certificate of Clinical Competence (CCC-SLP)
- Active CPR certification
- OASIS and home health documentation training
- Must be able and willing to learn to utilize a portable electronic field device. Must be able to manage electronic patient health information (ePHI) remote access in accordance with HCS policies and procedures.
- Valid driver’s license and access to an automobile during working hours that meet the State of New Hampshire registration regulations and auto liability insurance.
