We are actively recruiting a Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our expanding behavioral health team in Middletown, NY. Requirements
Key Responsibilities
- Perform thorough psychiatric assessments and mental status evaluations for residents in nursing homes.
- Establish accurate diagnoses and design personalized treatment plans tailored to each patient's needs.
- Provide ongoing medication management, making timely adjustments based on clinical progress.
- Work collaboratively with physicians, psychologists, social workers, nursing staff, and other healthcare professionals to coordinate comprehensive care.
- Deliver supportive counseling and psychotherapy to patients and their families.
- Educate residents and caregivers about psychiatric conditions, treatment options, and practical self-management strategies.
- Maintain precise and compliant documentation within the electronic health record (EHR) system.
- Participate in case reviews, interdisciplinary meetings, and quality-improvement initiatives that strengthen clinical outcomes.
- Active New York State Nurse Practitioner license with certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP).
- Ability to work independently while managing a consistent caseload.
- Strong diagnostic abilities and familiarity with psychopharmacology and evidence-based treatment approaches.
- Excellent communication skills and the ability to build rapport with patients, families, and care teams.
- Proficiency in electronic health records and basic computer applications.
- A compassionate, patient-focused approach to behavioral healthcare.
