Psychiatric Mental Health Nurse Practitioner (PMHNP) - Queens, NY
Our team is looking for Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our growing behavioral health network in Queens, NY.

Requirements
- Conduct comprehensive psychiatric evaluations and regular follow-up visits for residents in skilled nursing and post-acute care environments.
- Develop and adjust personalized treatment plans that meet each patient's unique mental health needs.
- Prescribe, monitor, and manage psychotropic medications, ensuring safety and therapeutic effectiveness.
- Work collaboratively with an interdisciplinary care team-including physicians, nurses, psychologists, and social workers-to coordinate holistic care.
- Provide therapeutic guidance, counseling, and education for patients and families to support treatment adherence and understanding.
- Maintain accurate and compliant documentation in the electronic health record (EHR) system.
- Participate in team meetings, case conferences, and quality improvement initiatives to promote best clinical practices.
- Active New York State Nurse Practitioner license with Psychiatric-Mental Health certification (PMHNP).
- In-depth knowledge of psychopharmacology and evidence-based treatment for geriatric populations.
- Excellent assessment, communication, and collaboration skills.
- Comfortable working both independently and as part of a team-based care model.
- Proficiency in EHR systems and general digital literacy.
