Requirements

• Conduct comprehensive psychiatric evaluations and regular follow-up visits for residents in skilled nursing and post-acute care environments.
• Develop and adjust personalized treatment plans that meet each patient's unique mental health needs.
• Prescribe, monitor, and manage psychotropic medications, ensuring safety and therapeutic effectiveness.
• Work collaboratively with an interdisciplinary care team-including physicians, nurses, psychologists, and social workers-to coordinate holistic care.
• Provide therapeutic guidance, counseling, and education for patients and families to support treatment adherence and understanding.
• Maintain accurate and compliant documentation in the electronic health record (EHR) system.
• Participate in team meetings, case conferences, and quality improvement initiatives to promote best clinical practices.

• Active New York State Nurse Practitioner license with Psychiatric-Mental Health certification (PMHNP).
• In-depth knowledge of psychopharmacology and evidence-based treatment for geriatric populations.
• Excellent assessment, communication, and collaboration skills.
• Comfortable working both independently and as part of a team-based care model.
• Proficiency in EHR systems and general digital literacy.
