From our heart to yours, we invite you to join the Kensington Family!SUMMARYThe Registered Nurse (RN), Support & Assessment Nurse collaborates with the Director of Nursing Services to conduct thorough Resident assessments, develop care plans, and coordinate nursing services. This role involves providing direct, intermittent nursing care and assessments in alignment with physician orders and individualized care plans.
The nurse ensures the highest standard of Resident care while ensuring adherence to all applicable federal, state, and local regulations. DUTIES & RESPONSIBILITIESPerforms initial evaluation visits and ongoing reassessments of Residents’ nursing needs by reviewing pre-admission medical records, gathering personal and health information, evaluating functional abilities, and identifying the appropriate level and type of care requires for each Resident. Initiates and updates individualized Resident care plans in collaboration with physicians, neighborhood Managers, and other healthcare professionals.
Completes and submits all required assessments – including reassessments, transfers, resumptions of care, discharges, and significant changes in condition – within company defined time frames. Conducts thorough skin assessments and implements preventative skin and wound care protocols, including pressure injury prevention, ongoing skin integrity monitoring, and early intervention strategies to support and maintain optimal skin health. Provides nursing services in accordance with established care plans and documents each Resident visit with clinical and progress notes.
Evaluates the effectiveness of Resident service plans, medication administration records, treatment records, and other required documentation, and directs updates as necessary. Processes physician orders, communicates Resident needs and changes in condition to physicians, and completes certification and recertification orders, as well as discharge summaries, in a timely manner. Coordinates services with external providers, including pharmacy, hospice, home health agencies, and rehabilitation services.
Communicates changes in Residents’ conditions and care needs to nursing staff to ensure continuity and seamless delivery of care. Meets with prospective and current Residents and their families as needed to discuss issues related to Resident care, solve problems, and promotes positive customer relations. Ensures problems and complaints are timely managed in an appropriate manner.
Escalates complex issues to the Director of Nursing Services. Leads and participates in Health Maintenance meetings and conducts conferences to regularly review Residents’ individualized service plans, identify changes in condition, and coordinate care plan updates with interdisciplinary Team Members. Reviews medical records and monitors quality-of-care indicators.
Identifies opportunities for improvement and develops action plans to enhance care quality and ensure regulatory compliance. Per guidance from the Director of Nursing Services, attends and participates in workshops, seminars, and training programs related to the health care industry. Participates in professional development activities as required.
Maintains professional certification.Serves as a representative of the Community and nursing department in professional and Community activities, fostering positive relationships with referral sources and service providers. Ensures that concerns and complaints are addressed promptly and appropriately. Reports any suspected or confirmed elder abuse, mistreatment, or neglect in accordance with policies, and maintains compliance with all applicable Federal, State, and local regulations.
Other duties assigned as needed by the Director of Nursing Services. KNOWLEDGE, SKILLS, AND QUALIFICATIONSCurrent Registered Nurse (RN) license in good standing in the state of practice. Minimum of 2-3 years of nursing experience, preferably in senior living, assisted living, long-term care, or a related healthcare setting.Bachelor of Science in Nursing (BSN) preferred.Thorough understanding of Federal, State, and local healthcare regulations and survey processes applicable to senior living communities.
Must be able to clearly and proficiently communicate with people of all ages while maintaining an energetic and positive attitude. Must be able to read and write English. Strong clinical assessment, critical thinking, and problem-solving abilities.
Ability to write reports, business correspondence, and clinical documentation. Effectively present information and respond to questions from Managers, clients, customers, and the public. Ability to manage multiple priorities in a fast-paced care environment while maintaining high standards of care and compliance.
Must demonstrate freedom from pulmonary tuberculosis verified by a health screening performed by a physician not more than six months prior to or seven days after employment. Must be criminally cleared by Department of Justice, FBI and/or applicable State Police via the Live-Scan process prior to initial presence in the facility.Ability to work flexible hours including weekends, evenings, and holidays as needed. Ability to stand, walk, and move throughout the Community for extended periods of time while performing job duties.
Ability to lift, move, or assist with items or equipment up to 25-50 pounds occasionally. Strong knowledge of geriatric care practices, chronic disease management, and Resident-centered care models.
