Job Description
Overview
This position is responsible for completing assessments, developing treatment plans, designing support strategies, and assisting with care coordination for individuals eligible for services. Provides supervision and is a member of an interdisciplinary team. Works collaboratively with the provider community.
Qualifications
- Master’s degree in social work, counseling, or closely related behavioral science field. The degree must be from an accrediting entity (CSWE, CACREP).
- One year post-licensure experience as an LISW, LPCC, or equivalent.
- Maintain active independent licensure from the Ohio Counselor, Social Worker, and Marriage and Family Therapist Board (e.g., LISW-S or LPCC-S).
- Obtain and maintain Service & Support Administration Supervisor Certification.
- Previous mental health experience in social work or counseling.
- Strong documentation skills.
- Motivated, organized, and detail-oriented.
- Knowledge of community resources is helpful.
- Ability to build rapport with clients and direct support professionals.
- Strong leadership and facilitation skills.
- Good problem-solving, written and verbal communication, and computer skills.
- Ability to work as part of a team and collaborate with diverse backgrounds.
- Report unusual incidents and major unusual incidents.
- Knowledge or experience of the Developmental Disabilities system is a plus.
Responsibilities (Summary)
- Supports a team of professionals working collaboratively to assist youth and adults with intensive behavioral needs.
- Completes discharge or linkage to other additional services as needed.
- Conducts thorough diagnostic assessments using interviews, direct observation, history review, testing, and data analysis.
- Collaborates with the individual’s team to develop and implement individualized treatment plans, monitor and adjust goals, collect and analyze data, and complete updates as required.
- Communicates with families, service providers, probation officers, case workers, schools, and other interested parties to exchange information during the treatment process and documents all participant-related contact in compliance with agency standards.
- Provides oversight and supervision to assigned staff by monitoring work duties, establishing/modifying work methods, schedules, and priorities; monitors TCM billing and elevates critical issues to the manager.
- Develops and maintains positive working relationships to educate on Summit DD’s behavioral support resources.
- Connects with service providers and families to identify gaps and determine treatment effectiveness.
- Works with provider staff and families to train and implement behavior support strategies.
- Provides crisis intervention services and is available for consultation.
- Facilitates referrals to internal or external resources as needed.
- Participates in agency and community committees and meetings as needed.
- Identifies training opportunities and coordinates training with appropriate staff.
- Maintains all required service documentation.
