Key Responsibilities
- Provide comprehensive primary care to geriatric patients in assisted living communities
- Diagnose, treat, and manage acute and chronic medical conditions
- Perform comprehensive physical assessments and wellness visits
- Develop individualized treatment plans
- Coordinate care with specialists, families, and facility staff
- Prescribe medications and monitor treatment effectiveness
- Maintain accurate and timely documentation
- Educate patients and caregivers regarding disease management and preventive care
- Deliver compassionate, relationship-driven healthcare focused on improving quality of life
Required
- Active Georgia Family Nurse Practitioner (FNP) or Nurse Practitioner (NP) license
- DEA license (or ability to obtain)
- Minimum of 3 years of clinical experience
- Strong clinical assessment and decision-making skills
- Excellent communication and interpersonal abilities
- Ability to travel to one of the designated service areas
