As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life.This role is perfect for a registered nurse who wants to practice the full scope of nursing while building deep, meaningful relationships.
This position will have you building report with patients, families, physicians, facilities, and other interdisciplinary team members. You'll have the autonomy to coordinate comprehensive care for your caseload while working collaboratively with our interdisciplinary team to address the physical, emotional, and spiritual needs of those in your care.LocationOffice Location: Grand Rapids, MIMain Service Area: North Grand Rapids and Newaygo countiesThis role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homesHoursMonday to Friday, 8 a.m. to 5 p.m.Weekend On-Call RotationRequired QualificationGraduate of an accredited Diploma, Associate or Baccalaureate School of NursingCurrent State License as a Registered NurseMinimum one (1) year experience as a nurse in a clinical care settingReliable transportation, valid driver's license, and valid automobile insurance coveragePreferred QualificationsBachelor of Science in Nursing (BSN) Hospice or home health experienceEssential Skills and CompetenciesNursing skills as defined by accepted nursing standardsDemonstrates ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environmentDemonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestylesAbility to handle multiple priorities, documentation requirements and deadlinesPay Range $80,000—$85,000 USDHow We Care for YouMinimum of 3 Weeks Paid Time Off (PTO) Medical, Dental, and Vision InsuranceHSA and FSA options including Dependent CareCompany paid Short Term DisabilityCompany paid Life Insurance 401(k) with Employer MatchMileage ReimbursementCompany Vehicle Program for field rolesPet InsuranceID and Fraud ProtectionAnd more...Background ScreeningEmployment is contingent upon the successful completion of a background check.
