LICENSED PRACTICAL NURSE

Heritage Park Care Center seeks a compassionate Licensed Practical Nurse to deliver high‑quality, patient‑centered care in our long‑term and post‑acute setting. The LPN will administer medications and treatments, monitor vital signs, document accurately in electronic health records, and collaborate with RNs, physicians, and interdisciplinary teams to implement care plans. Responsibilities include wound care, resident assessments, support with ADLs, patient and family education, and maintaining infection‑control and safety standards.
