LPN shall act as an active member of the interdisciplinary team conducting at least, but not necessarily limited to; assessment and screening, participation in team conferences, case management, discharge planning, intake and referral and other clinical duties. The LPN will provide a valid and current Louisiana license prior to hire.

• Demonstrates knowledge of Patient and Family Education.
• Identifies problems and needs of clients and develops a nursing plan designed to meet clients’ needs.
• Assist Medical Director with verbal and written physician orders.
