Occupational Therapist

CenterWell Home Health is seeking an Occupational Therapist to provide compassionate in-home therapy to seniors and adults with chronic conditions. In this role, you will perform comprehensive evaluations, develop personalized treatment plans, and deliver evidence-based interventions to improve patients’ safety, function, and independence at home. You’ll collaborate closely with an interdisciplinary team, educate caregivers, and document care in line with clinical and regulatory standards.
