Our client, a fast-growing healthcare technology company transforming how self-insured employers navigate surgical care, is seeking a Patient Care Coordinator to join its growing team.
Position Overview
This role is ideal for an active LVN with at least 2 years of experience in a healthcare startup or other fast-paced care delivery environment. As the first point of contact for patients, youll play a critical role in guiding individuals through enrollment, coordinating care logistics, and ensuring a seamless experience throughout their treatment journey.
The successful candidate is empathetic, detail-oriented, and passionate about delivering exceptional patient experiences. Youll work closely with patients, providers, and internal teams while leveraging innovative AI-powered tools to streamline workflows, improve communication, and support high-quality care coordination. What youll do
Make 50–75 outbound cold calls per week to engage and enroll eligible patients into their surgical benefit
Manage a caseload of patients through onboarding, eligibility verification, and medical record collection
Coordinate handoffs between patients, nurses, providers, and the operations team
Contribute to AI scripting and process improvement by sharing insights from patient interactions
Work closely with the executive team on business metrics and operational improvements
Required qualifications
2–7 years of experience in remote healthcare care coordination, patient engagement, or case management
Active LVN license (any state — this is a non-clinical, care navigation role)
2+ years of remote work in healthcare (care coordination, case management, or patient engagement)
Comfortable working from home 70% of the time
Comfortable with outbound patient engagement and cold calling (~50–75 calls/week)
Startup or health tech experience with demonstrated business acumen and comfort with direct feedback
Tech-savvy and genuinely excited about AI — not intimidated by rapidly evolving tools and processes
Fluency with GSuite, Salesforce, and digital health platforms
One of the following is REQUIRED
Currently at a health tech startup in care coordination or patient services (e.g., Carēm, Lantern, Transcarent, Included Health)
Remote nurse in case management at a payer (e.g., UnitedHealthcare, Aetna) actively pursuing business or tech credentials (informatics degree, HBS courses)
Nice to have
Familiarity with surgical care pathways (ortho, spine, bariatric, cardiac)
Bilingual in Spanish or Hindi
