Job Summary

Seeking a Behavioral Health Specialist II (LICSW) to provide clinical social work and behavioral health services for older adults enrolled in a PACE program. This role is responsible for psychosocial assessments, counseling, case management, crisis intervention, caregiver support, treatment planning, and resource coordination.

Responsibilities

• Conduct psychosocial assessments and behavioral health evaluations
• Provide individual counseling and behavioral health interventions
• Develop and implement treatment plans
• Coordinate case management and community resources
• Provide crisis intervention and caregiver support
• Collaborate with interdisciplinary team members on participant care plans
• Educate participants and families regarding available services and support
• Advocate for participant needs and access to services
• Maintain accurate and timely documentation

Requirements

• Master's Degree in Social Work (MSW) from an accredited institution
• Active Washington State Licensed Independent Clinical Social Worker (LICSW) license
• Minimum 2 years of behavioral health or clinical social work experience
• CPR and First Aid certification (or ability to obtain within one month of hire)
• Valid driver's license and ability to travel throughout the Seattle area
