Primary Care Physician-OSH

Role: Primary Care Physician responsible for delivering equitable, value-based healthcare to Medicare patients in a neighborhood primary care setting.

Responsibilities: Assessing, diagnosing, and managing patient care; prescribing medications; maintaining electronic medical records; collaborating with leadership to meet quality goals; overseeing care teams.

Qualifications: MD or DO with board certification or eligibility in Internal Medicine or Family Medicine; active, unrestricted license and DEA; US work authorization.

Preferred Skills: Geriatrics fellowship, experience with older populations, outpatient primary care, and value-based care models; bilingual skills are a plus.
