Program Coordinator

Carle Health is seeking an RN Program Coordinator to support high-quality, community-based care within our Healthcare & Medical Services department. In this role, you will coordinate clinical programs, perform comprehensive patient assessments, and develop individualized care plans. You will collaborate closely with physicians and interdisciplinary teams to monitor outcomes, support quality improvement initiatives, and ensure regulatory compliance.
