About the Role
We are seeking a compassionate and experienced Registered Nurse to join our team. This community-based role provides the opportunity to deliver safe, high-quality, and person-centred nursing care to older adults and individuals requiring support to maintain their health, wellbeing, and independence at home.
Reporting to the General Manager Aged Care, you will work collaboratively within a multidisciplinary team to assess clients' needs, develop and implement care plans, and deliver evidence-based nursing interventions. You will play a key role in improving client outcomes while building meaningful relationships with clients and their families.

Key Responsibilities
- Conduct comprehensive nursing assessments and reassessments of clients' physical, cognitive, and psychosocial needs.
- Develop, implement, and evaluate person-centred care plans and interventions.
- Deliver evidence-based nursing services, including medication management, wound care, pressure care, dementia screening, and continence support.
- Educate clients, families, carers, and support workers on care plans and interventions.
- Collaborate with multidisciplinary teams and make timely referrals when required.
- Maintain accurate documentation, reports, clinical records, and billing requirements.
- Monitor client wellbeing and respond appropriately to changes in condition or escalating concerns.
- Support quality improvement initiatives and contribute to the development of clinical practices and procedures.
- Provide guidance and mentoring to colleagues and contribute to a positive and collaborative team environment.
- Ensure compliance with organisational policies, professional standards, and legislative requirements.
About You
- Current AHPRA registration as a Registered Nurse.
- Minimum one year of post-graduate experience in Acute Care, Residential Aged Care, NDIS, community health or similar.
- Strong clinical assessment, care planning, and evidence-based practice skills.
- Excellent communication and interpersonal skills with a person-centred approach to care.
- Strong organisational and time management skills with the ability to manage multiple priorities.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Proficient in computer systems and maintaining accurate clinical documentation and reports.
- Demonstrated commitment to continuous learning, quality improvement, and professional development.
- Compassionate, proactive, and committed to delivering exceptional care and outcomes for clients.
- Current Working with Children Check, National Criminal History Check, COVID-19 vaccination, and an unrestricted Australian Driver's Licence (P2 licence acceptable).
