RN Care Manager II

As an RN Care Manager II at Mercy Hospital St. Louis, you will coordinate complex inpatient care, ensuring safe, efficient transitions across the continuum. You’ll perform comprehensive assessments, develop and manage evidence-based care plans, and collaborate with physicians, therapists, social workers, and community partners.
