RN Case Manager
Location
Portsmouth, VA – Hybrid (remote & field). Travel 3–4 days per week within an assigned territory. Coverage includes Portsmouth and surrounding areas. Territory assignments may change based on business needs.
Schedule
Monday – Friday. Flexibility to accommodate patient needs and nephrology partner availability.

Responsibilities
This position functions autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost‑effective care while minimizing fragmentation of the healthcare delivery system for ESRD and CKD patients. The role coordinates care between the patient, family, physician, provider and care teams, community, and Regional Operations Manager.
Additionally, the RN Case Manager:
- Assists patients, families and care teams in navigating the healthcare system
- Develops and manages individualized care plans
- Coordinates services across interdisciplinary teams
- Supports patients through complex healthcare transitions
- Collects and analyzes data to improve clinical, financial and patient satisfaction outcomes
- Acts as a change agent to improve processes and quality outcomes
- Provides coaching and performance feedback to healthcare staff
- Maintains communication with nephrology partners and internal stakeholders
Qualifications
- A.D.N degree from an accredited school of nursing required; B.S.N preferred; a three‑year diploma from an accredited program may be substituted for a nursing degree
- Current Registered Nurse (RN) license in the state of practice required
- Continuing education credits maintained as required by state of practice
- Minimum five (5) years’ experience in clinical nursing strongly preferred
- Minimum three (3) years’ experience in renal nursing preferred
- Case‑management experience required. If no case‑management experience, a minimum of three (3) years of experience assessing patient needs, developing and managing individualized care plans, coordinating services across interdisciplinary teams, and supporting patients through complex healthcare transitions is required
- Certified Nephrology Nurse (CNN) or Certified Case Manager (CCM) preferred
- Demonstrated knowledge of data and clinical, financial, and patient satisfaction outcomes
- Demonstrated experience as a change agent
- Demonstrated knowledge of CQI techniques
- Previous experience in healthcare performance coaching required
- Current CPR certification required
- Ability to adapt practice to new electronic processes and productivity expectations as it relates to Capella implementation
- Valid driver’s license with a positive driving record and insurance coverage as required
- Basic computer skills with MS Word and Outlook proficiency
- Functional proficiency with DaVita clinical software, including Capella, required within 90 days of employment
- Home office with internet connectivity at minimum 1 MB upload and 1 MB download speed required
Benefits
