Scheduler - Home Health Full‑time, Associate’s degree, located in Stockton, CA. What You’ll Do Coordinate and manage clinician schedules for patient home visits Utilize Homecare Homebase (HCHB) to create and maintain weekly scheduling Communicate with clinicians, patients, and leadership regarding scheduling needs and changes Assist with coordination of patient visits to support continuity of care Provide administrative support including data entry, filing, correspondence, and reporting Answer incoming calls and route messages professionally and efficiently Maintain accurate scheduling and patient records Support branch operations and assist with additional office duties as needed Who We’re Looking For You are organized, proactive, detail‑oriented, able to manage multiple priorities in a dynamic healthcare setting, enjoy problem‑solving, support clinicians, and help ensure patients receive timely care.
