Social Work Care Manager Outpatient Master of Social Work (Hiring Immediately)

Position Summary As part of the outpatient multidisciplinary team, provides assessment and intervention to assist clients/families to improve social and economic difficulties interfering with health and wellness through use of casework process and principles, strategies, and community resources. Receives referrals for individuals from at‑risk populations from interdisciplinary team members within the outpatient clinic setting.

Responsibilities

Collaborates with the outpatient multidisciplinary teams to determine the nature of the client's situation by interviewing the client; assessing medical, psychological, emotional, and social information; making on‑site visits.

Establishes a course of action by exploring options, setting goals with the client.

Obtains assistance for the client by referring him/her to community resources; arranging for appointments; establishing rapport with other agencies.

Fosters the client's action or adjustment by interpreting attitudes and patterns of behavior, explaining and pointing out new options as patients are presented with new diagnoses and treatment options within the outpatient clinic setting.

Performs individual and/or family therapy, as indicated.

Qualifications Required

Master’s degree in social work from an accredited educational institution.

State licensure or eligibility for licensure as a Master Social Worker (LMSW) and maintenance of continuing education requirements.

Preferred

Basic Life Support (BLS) certification as a Healthcare Provider by the American Heart Association, American Red Cross or equivalent through the Military Training network (MTN).

Oncology or medical social work experience.

Internship completed at a medical facility.

Certification in Case Management Certification (ACM or CCM).

Three years of acute hospital care or social work experience.
