Job Summary

We are seeking a dedicated and compassionate Social Worker with at least 2 years of LTC experience to join our team at Avir at John Jones in Burleson, TX. • Provide social work services to patients and families, including assessment, counseling, and education to meet their psychosocial needs

• Identify and address social determinants of health that impact patient outcomes

• Participate in quality improvement initiatives to enhance patient care and satisfaction

• Keep accurate and timely records of patient interactions and progress
