Social Worker , Case Management (Hiring Immediately)

Employment Details Part‑time, rotating shift. 12‑hour shifts (8 a.m.–8 p.m. or 8 p.m.–8 a.m.). Available any day of the week, including weekends. Additional shifts may be picked up at Mount Carmel St. Ann's. Position Overview ED Social Worker (PRN/Casual) at Mount Carmel Dublin. The role supports the Emergency Department and assists on the inpatient unit as needed. Position Purpose Mount Carmel Dublin Case Management facilitates communication and care coordination along the continuum through effective transitional care management utilizing a multidisciplinary team. The Emergency Department Social Worker intervenes with patients who have complex psychosocial needs, provides assistance with eligibility determination for social programs and funding sources, and qualifies for community assistance from a variety of special funds and agencies. The worker offers crisis intervention, coordinates and facilitates the development of a discharge plan of care for high‑risk patient populations, conducts lethality assessments, and refers to inpatient psychiatric facilities when indicated.

The Social Worker will also assist other Mount Carmel Health System Emergency Departments virtually through our Telehealth platform. Responsibilities Intervene with patients who have complex psychosocial needs. Assist with eligibility determination for social programs and funding sources.

Provide crisis intervention to patients and families. Coordinate and facilitate development of discharge plans of care for high‑risk patients. Conduct lethality assessments and refer to inpatient psychiatric facilities as necessary.

Provide virtual assistance to other Emergency Departments via Telehealth. Support the inpatient unit as needed. Minimum Requirements Master's degree in Social Work.

Current LSW license to practice in the State of Ohio. Minimum 1 year of hospital experience with acute crisis intervention and behavioral health assessment. Effective communication, self‑directed and adaptable to rapidly changing regulatory requirements.

Excellent clinical assessment and analytical skills. Computer competency and comfort with virtual telehealth platforms. Effective interpersonal skills, ability to work across departmental boundaries, facilitate problem resolution, and maintain a professional demeanor in difficult situations.

Knowledge of the functional operations of utilization management, discharge planning, care coordination, and transitions of care.
