Case Manager – Rosemount Integration Support Program (FT Permanent)
The Canadian Mental Health Association is looking for a Case Manager to support residents within the Rosemount Integration Support Program. The program operates 24/7, staffed by a multidisciplinary core team, including a Manager, a Case Manager, and a Transitional Housing Support Worker. The Case Manager collaborates with industry partners to provide employment supports, peer support, nursing, and primary care, ensuring comprehensive and coordinated care.
Responsibilities include supporting residents’ mental health and social care needs, promoting housing stability, recovery, harm reduction, and community integration while respecting tenant autonomy and dignity.
Salary: $54,124 – $62,733
Hours of Work: 35 per week
Responsibilities
- Provide on‑site case management support and develop personalized care and relapse prevention plans.
- Deliver mental‑health counseling services, including assessment, information, orientation, and housing support at the Rosemount location.
- Facilitate internal and external referrals for services not provided by the program, including specialized supports.
- Conduct comprehensive assessments to understand clients’ needs, strengths, and goals and implement individualized service plans in collaboration with clients and their support networks.
- Organize, plan, and facilitate group and individual sessions for skill building, life‑skills training (e.g., household maintenance, budgeting), and other client needs.
- Develop and implement community outreach plans to promote program services.
- Collaborate with clients, community partners, and relevant agencies to create individualized case management plans, ensuring access to primary care, employment resources, and navigation of health, social care, and legal systems.
- Support clients through individualized case management, life‑skills coaching, and service planning to promote successful transitions to independent living, including safety and long‑term housing plans and post‑transitional support.
- Build and maintain a trusting, respectful, collaborative relationship with clients and their support systems and provide supportive counseling for a range of issues and psycho‑education around illness, medication, and symptoms.
- Advocate on behalf of clients and connect them with community resources, establishing effective relationships with staff of other agencies.
Qualifications
- Undergraduate degree or diploma in social services or a related field.
- Minimum of two years of direct service experience in social work or community work with adults with psychiatric disabilities.
- Knowledge of crisis intervention/prevention, addiction issues, recovery‑oriented approaches, cultural competence, trauma‑informed care, and severe and persistent mental illness.
- Experience with refugee/settlement challenges or mental health and addiction issues (asset).
- Ability to work flexible hours, including evenings and weekdays, and to manage crises in a trauma‑informed, strength‑based environment.
- Strong analytical, assessment, and decision‑making skills, able to work independently and within a multidisciplinary team.
- Training in trauma‑informed care, family‑centered practices, culturally competent care, and motivational interviewing.
