Job Description
Join LOFT Community Services as a Case Manager working with French-speaking clients in need of support for mental health challenges. This onsite position focuses on hands-on case management to elevate clients’ quality of life.
As a key member of the team, the Case Manager collaborates with clients to develop comprehensive and personalized recovery plans. This includes guiding clients through essential community resources and ensuring they receive adequate healthcare. You will also focus on teaching life skills while advocating for clients' needs effectively.
Key Responsibilities: • Provide direct support and assistance through community contact • Implement personalized Client Safety Plans and OCAN assessments • Liaise with support networks to enhance client services • Assist clients in resource access for essential needs • Maintain clear documentation and case notes in client databases
Requirements: • Minimum of three years’ relevant experience in social services • Excellent French communication skills, both spoken and written • Familiarity with mental health resources and rehabilitation • Proven problem-solving abilities and interpersonal skills • Must have a valid driver’s license for client transport
