Help support young adults in achieving their goals through comprehensive case management. As a key member of the Community Programs team, you’ll perform intake assessments and provide tailored support to 2SLGBTQ youth aged 14-29.Your role will involve collaborating with various partners, developing Smart plans, and ensuring access to essential health services.

Strong interpersonal skills and crisis management abilities will be vital.
