WELL Health Clinic Transformation Specialist

Elevate healthcare delivery as a Clinic Transformation Specialist with WELL Health in Ontario. This full-time role centers on successful clinic engagement and digital tool adoption.

Working closely with approximately 22 primary care clinics, you will implement and manage digital health solutions that enhance patient care. This position involves daily hands-on interactions, fostering relationships with physicians, and ensuring performance targets are met across your portfolio.

Key Responsibilities: • Direct digital transformation efforts for selected clinics • Manage go-live processes for digital health solutions • Serve as the main point of contact for clinic teams • Assess and report on clinic performance metrics • Support digital patient engagement initiatives

Requirements: • 2-4 years of experience in healthcare client-facing roles • Understanding of Ontario FHO clinic operations • Proven ability to influence behavior change • Strong interpersonal skills with healthcare providers • Willingness to travel to clinic locations
