Job Description
Key ResponsibilitiesDeliver case management services and counselling support to youths residing in a residential care environment.Build and maintain effective working relationships with external organisations, partners, and stakeholders, while sharing necessary updates when appropriate.Organise and manage residents’ external medical, counselling, legal, or agency appointments.Evaluate the needs and risk factors of residents and their families, and develop suitable Individual Care Plans (ICP).Oversee the progress of each care plan and ensure agreed interventions are carried out together with care staff and programme teams.Provide family counselling and facilitate reconciliation or relationship rebuilding between residents and their families.Track residents’ daily conduct, behavioural incidents, and personal development progress.Participate in weekly disciplinary review meetings, provide professional recommendations, and follow through on agreed outcomes.Keep proper, accurate, and updated case notes and resident records.Compile, review, and submit scheduled reports as well as ad-hoc reports when required.Facilitate group sessions for residents and/or family members to strengthen practical life skills and emotional resilience.Assess proposed external workshops, classes, and talks to determine suitability and benefit for residents.Coordinate with relevant agencies, internal departments, and programme teams to arrange approved activities or sessions.
Requirements:Degree in Social WorkRelevant experience in casework, counselling, youth work, social services, or residential care settings preferred.Strong interpersonal and communication skills with the ability to engage residents, families, and external stakeholders.Able to conduct assessments, develop care plans, and manage case documentation accurately.Experience in family counselling, conflict resolution, or restorative practices will be an advantage.
